Assistant Coach Evaluation/Feedback

This online evaluation is intended for the sole purpose of providing constructive feedback to our coaching staff in helping the DYHA Coaching Coordinator and Board of Directors to identify any shortcomings.

This is a volunteer program which strives to provide you and your child(ren) the most enjoyable youth hockey experience possible. With your valued input, we will continue to help make this a fun learning experience for all of our entire Demon’s members. Your participation is appreciated and critical to providing constructive feedback, positive or otherwise. Please fill out in the manner you feel appropriate, providing your feedback for an individual or individuals.
